Two options available to schedule your test

Option 1 – To schedule a test or service online, enter your location zip code below and select your preferred testing center location to proceed to the registration / check-out page. A registration/ confirmation form with instructions, testing center address and hours will be sent to the e -mail address you provide. No appointment is necessary, however a test registration is required. Testing centers do not accept any form of payment, payment must be received at time of registration. Not all testing centers are listed, please call for additional locations. If you select a preferred testing location, please be aware that not all testing centers provide all drug and breath alcohol tests. In the event that a preferred testing center does not offer the test you have ordered an alternate testing center will be selected for you.

Option 2 – You may also call Health Screenings USA Inc at (800) 219-7161 to speak with a customer service representative and schedule your test by phone. Please have the customer information including zip code, preferred location and method of payment available.
